Total and permanent disability
Use this online form if you've been diagnosed as totally and permanently disabled and can't go back to work.
Income protection
Use this online form if you can't work because of serious illness or injury.
Terminal medical condition
Use this online form if you're diagnosed with a terminal medical condition.
Make a death claim
If you expect to receive someone's death benefit, including their super and any insurance, please let us know.
